I don't know if the issue is in macOS or iOS or both, but when I went from the US to Australia, and the computer automatically updated the time zone, birthdates on all of my divers advanced by a day. Manually changing the time zone on macOS reverted the dates back a day. My assumption is behavior is the same on iOS.
Do you use the birthdate to calculate anything? If not, I'd suggest making birthdates static.

No, it's just for your personal record and isn't used anywhere other than that field.
Thanks for the report. I've added a note to look in to it, though it's pretty far down my priorities at the moment. The same will be true of your gear purchase dates, service dates, etc.
